Companies who would like to create an account will enjoy great discounted pricing. Simply type your information in the electronic form fields, save, and email to info@high5.biz where we will then receive it. We will contact you to review information and discuss pricing. We recommend keeping the form in the electronic format so that when pricing is discussed, it can be entered into the appropriate fields and so you only need to print the form once for your records. This is when you will sign the bottom of the form.

This will save you time when visiting our office as well as help prepare you for information you may need to know. If you are unsure of what to fill out, just fill out the personal information section and where to send results. You do not have to enter your SS# until you are in the office.

If you would like a copy of your background check, please read this form for directions, print and complete following document. If your most recent background check was completed within 30 days, there is no charge and you may fax the form to the number provided. If your last background check was completed later than 30 days, there will be an $8 fee that must be mailed in with the completed form. Cash will not be accepted. Details are listed in this file.
